The easiest hack of this kind is to simply prevent the on chip EEPROM being written, so they buy an official card, destroy the EEPROM write circuit, and use it on lots of meters. hence the need to go door to door. It could also be a cloned card, but that is a little harder, because you now need to get the cloned chips manufactured. Since the meters cannot communicate with each other, they have no way of knowing that the cloned card is being used in multiple locations.
Ideally the meter should not add the value until it verifies that the card has subtracted the value (i.e. decremented the on chip EEPROM) but some simple changes to the code can make this communication happen with the RAM rather than the EEPROM. (BTW Most modern payment cards can prevent this by encrypting RAM data differently from EEPROM data)
3) Fixes. All fixes for this problem require that a valueless card be inserted into the electricity meter and have the meter sign the cards info so that the value subsequently added can be uniquely created for this meter. Unfortunately this is a two step procedure for the end customer and is is therefore unpopular. An alternative is to have the meter display an ID which can be written down and entered when the credit is purchased. Unfortunately this puts people in the crypto loop, so this system is prone to error.

Decent point. I had a hell of a reply for you too. I spent an hour designing an online smart meter system that used signed messages stored on untrusted cards along with a message revocation list at utility side to provide the security with no usability loss for customers. After I wrote up two great systems with tons of capabilities, I came up with a radical idea that seems so good I deleted all the other stuff.
